Claim for Damages

If you would like to submit a general liability claim, you must submit your Claim for Damages Form (PDF) to the Burbank City Clerk's Office. 

You may email your Claim for Damages Form and any additional documents to [email protected] or you can submit it via U.S. Mail to the below address:
